The Dutch 'Guidelines for the use of antipsychotics in schizophrenic psychosis' ('Richtlijn antipsychoticagebruik bij schizofrene psychosen') differs, without given reason, on some essential points from its only source of reference, the 'Practice guideline for the treatment of patients with schizophrenia' (apa1997). As a result of these modifications the target group is less clearly defined and the duration of the stabilisation phase is doubled.
The developments from 1997 onwards are not integrated into the Dutch Guidelines. According to recent views, the dosage recommendations are too high for patients who experience their first psychosis. The Dutch Guidelines also shows a lack of nuance regarding the indication for maintenance treatment in first onset psychosis.
The strictness of the Dutch Guidelines could have serious consequences in cases where the determination of professional liability is an issue.
