A patient is described who was treated under the diagnosis schizoaffective disorder. According to dsm-iv now a bipolar disorder with mood-incongruent psychotic features should be classified. The historical backgrounds of this change are discussed. In view of this patient it is stated that with the broadening of the category of bipolar disorder at the cost of the category of schizoaffective disorder the classification problems in this area have not been solved.
